susy,a 28-year-old female has had a 9-year history of periods of “feeling great and feeling lousy”. During the “feeling great” periods, she reported increased libido and decreased sleep. During her “feeling lousy” periods, she reported decreased concentration and tearfulness. she does not have decreased appetite or decreased libido during the feeling lously periods. what is the most likely diagnosis?
a.seasonal affective disorder
b.moodfluctuation disorder
c.dysthymia with depression
d.double depression.
e.atypical depression
f.bipolar disorder type I
g.bipolar disorder type II

It has to be Cyclothymia as the symptoms of Hypomania and Depression have been present since 9 yrs. So I would go with Cyclotthymia even though it is not given as the choice.

it's hard to differentiate mania and hypomania, depression and dysthymia. In this case, I think the patient has hypomania and depression/disthymia. but can the length of the persistence of the symptoms serve as a diagnostic criteria too? I mean, bipolar can be quite long. please correct me if I am wrong. vysserx, please kindly tell us the correct answer. :roll:

the answer is bipolar type II. THE EDUCATIONAL OBJECTIVE of this question is to make clear the difference between the BIPOLAR I and II . the difference is SEVERITY. if bipolar I is WAYUP AND WAYDOWN, II is littleup and little down. SUSY, has episodes of feeling great wihch do not qualify for MANIA. feeling lousy episodes do not qualify for episodes of depression. so, THERE IS A CLEARCUT ANSWER , BIPOLAR TYPE II.

bye the way, where does CYCLOTHYMIA fit in? well, CYCLOTHYMIA is a milder variety of bipolar type II. TO AVOID confusion, i did not give that as choice.hope this helps!

It is given in the Kaplan lecture notes thst similar kind of question and the answer is Cyclothymia. That is how I thought it is the answer. U may please reffer to Kaplan lecture notes. Correct me if I am wrong.
