indication of exploratory laporotomy vs DPL



please explain

After trauma if
  1. -Shock with sign/Symptoms of Peritonitis
  2. -Shock with abdominal injury not improving with Sufficient fluids

Are definite indications of straightforward laprotomy.

  • practically the only indication of DPL these days is in instable (but not in shock) patient in which we are not sure the reason for low BP is intra-abdominal hem , also sometimes it may be used for Dx of hollow organ rupture when other tests are inconclusive in a stable patient .It is done bedside and in instable patient in OR.


  • In stable patient always go for CT.


  • In patient in shock with no other source of Hem or peritonitis go directly for laparotomy.


  • gunshot ---------> always laparotomy even in stable patient

SURE PERFORATION : exploratory lap (gunshot)

NOT SURE IF PATIENT BLED INTO ABDO WITH UNSTABLE PATIENT : DPL or sono

NOT SURE IF PATIENT BLED INTO ABDO WITH STABLE PATIENT : CT
