21yr old male presented to the ER with extreme muscle weakness in the arms and legs,he didn’t remember any similar attack in the past,his PMH was only positive for smoking,and experience of the stiffness in muscles of the hands in cold weather.After sending a blood sample to the lab,treatment with IV KCl was immediately started,but the paralysis became more severe and very quickly he was unable to talk and breathing became difficult.The most accurate statement about the possible diagnosis in this pt is:
a:meals high in carbohydrate or sodium can provoke attacks
b:chronic administration of acetazolamide may prevent the attacks
c:the primary defect is in Ca channels
d:the decrease level of TSH is a common finding

looks like hyperkalemic periodic paralysis
Weakness occurs during rest after a period of strenuous exercise or during fasting. It also may be provoked by potassium, cold, ethanol, carbohydrates, or stress.
