Hypoxia

Aggravated by sitting up

Relieved by lying down

DX = ??

ORTHODEOXIA

"increase right-to-left shunting either intrapulmonary or intracardiac with upright position". It may or may not be associated with PLATYPNEA.

1. Hepatopulmonary syndrome (Intrapulmonary shunting) Changing from a supine to upright position can result in preferential perfusion of basilar regions with increased shunting and hypoxia



2. interatrial shunts (PFO): postural hypoxemia seems a consequence of redirecting flow in the IVC towards the interatrial septum due to the distortion of anatomic relations
3. Pumonary embolism: I've seen case reports with chronic PE, multiple PE or large PE

Large PE: a case in Braunward's

Multiple PE: a case in one of the textbook when I search for orthodeoxia

I will try to look for evidence with explanation and post them
