2 year old child presents with weight loss, irritability, and has a chronic productive cough. He has no fever and lung exam reveals rhonchi, wheezing, and rales. Chest xray shows patchy infiltrates and atelectasis. Upon gram staining, gram negative motile curved rods are grow aerobically are seen. The organism causing the pneumonia in this kid is also responsible for : A)Otitis externa B)Croup C) Epiglotittis D)Otitis media E)Meningitis

is it H. influenza? it causes meningitis & epiglottitis too.

H. influenzae is directly responsible for epiglottitis, but a minor cause of meningitis.

Isn't Haemophilus non-motile? I think it might be Pseudomonas aeruginosa, which would be associated with A) Otitis externa.

I dont have the answer to this question, but I thought Pseudomonas as well because the patient has cystic fibrosis, and pseudo is responsible for pneumonia in these patients and just like sakaki said, Otitis externa would then be another complication. If anyone knows something else, correct us please.

yea, it is Pseudomonas Aerogenosa.... Pneumonia in the cystic Fibrosis patient..the first we should think of is Pseudomonas Aeroginosa..Gram negative, aerobic definietly says ' Yes it is'.... Next ..the disease among the listed caused by the same guy..is " Otitis Externa"..recall..also called "Swimmers Ear".... Also the other conditio where Pseudomonas cause "Otitis Externa..is "Diabetic" patients..as well !!

But the other point here is if u want to real out ' Otitis media' the most common causes..are Streptococcus Pneumoniae and H. Influenzae..even in cases of external ear infections with P. aeruginosa the middle ear is typically spared.. Then Croup..parainfluenza.. Epiglottitis..H. Influenzae.. Meningitis..numerous causes..if we consider the age..then H-influenzae..N.meningitidis.. Hope this helps! But if this P.aeroginosa..is motil ecurved rod..I should still look..will get back if I get it..: )

I agree with you drkalsam, it has to be otitis externa, its the only one that makes sense.

A)Otitis externa - it is Pseudomonas Aerogenosa (motile rode, but don't know about curved)

Yeah, Pseudomonas aeruginosa isn't really curved, but I suppose that it's an adequate description.
