Is the diagnosis made only by autopsy? and before that are the clinical signs and symptoms are the only way to make the correct diagnosis.

Diagnosis of AD is mostly clinical with Lab tests being normal and MRI showing diffuse cortical atrophy. Labs / imaging is done mainly to rule out other causes of dementia. A clinical diagnosis of AD is confirmed at autopsy 90% of times.

One of the challenges of trying to diagnose Alzheimer's disease is that it doesn't cause big changes that are visible on MRI and CT scans. So PET imaging has a great advantage because it shows a specific pattern that differs whether a person is experiencing normal aging, Alzheimer's disease or other kinds of dementias. PET scans has a diagnostic sensitivity of 91%, and specificity of 75 %.
