Hi...Today I printed my certification of identity form along with certification of post graduate training form. My q is this: Should I just send CID or should I sent 2 forms to fsmb. In the certification of post graduate training form ask about hospital name however I am not a resident now. What should I write to that section. Please help me, because I want to apply for this year. Many thanks...
