Dear bbb, for a FMG, there are so many things to consider... Still, my most important goal is a nice program with outstanding teaching. I need visa. Should I "forget" the top hospitals, because I am a FMG w/o visa? People say that one must be a very strong FMG to get there (=for example research at the goal program + publications + one year USCE + 4 LORS +...). It sounds easier to win the Nobel prize than to match there. I am interested in Cardiology. Should I look for hospitals that offer Cardiology fellowships? Is it so difficult to get Cards fellowship after a community hospital residency? Regarding scores: do the PD distinguish e.g. 99x2/250x2 and e.g. 99x2/272x2? If you both scores are high, say both +270, will it make a difference? Perhaps some questions may sound easy for American applicants, but when you live abroad, you may consider things in different light. Thank you. Best regards and Gruesse aus Deutschland , cmuj

If u have stellar scores and u have something special ahead of the American candidates who apply to this same top hospitals then Visa shd not be an issue. harvard, JHopkins etc want the best irrespective of where u are from so visa sponsorship shdn't be a problem U shd analyse yourself. Top american candidates from harvard,Cornell...with publications and research and honors will apply to John Hopkins. Do u match up??? Sometimes u have to be realistic. If u have what it takes never allow anybody in an Internet Forum tell u that u cant achieve something

Thank you for your answer, truemamamia. I am relatively new here and I have not read many posts yet. “FA” says that it is reasonable to divide the programs into 3 categories: dream/very nice/acceptable. Every year there are many very, very good FMG, who surely apply to some “dream programs”. Probably the top programs do not plan to have more than max. 10% FMGs among their residents. I think that every year there are only a few “dream spots” for FMGs per program. If most of really good FMGs apply to the “top 20 programs” (1/3x e.g. 60), than it must be like 1:????? per spot…it must be … almost impossible. I applied to 60 programs. I decided to send a few applications to the “top” programs. Still, my goal is to match to a nice, strong program, and I am sure I would happy in any program, which helps to unfold residents’ abilities. I think you are right, the top programs will sponsor any visa (or at least J1) if they really like a candidate.

Any program which is dominated by FMGs is looked at suspiciously. But I don't think that is the biggest Factor. Places like Harvard,John Hopkins,Cornell have applications from the best students from across the World. And I think they interview top top students. But by the end of the day u have to bear in mind that the top most American applicant is a student at a place like Harvard(just ex), has letters from maybe somebody who won a Nobel Prize, has published in an American Journal and has many honors(most foreign schools don't have this).Unless maybe u have done something extraord like start a Clinic in a 3rd World xtry...National Publications and Honors Apply broadly. For example if u have >250 u are a stellar applicant, u shd never rule yourself out of Harvard. Sometimes when u get to the interview and you talk they know u better and they can say wow this guy has the potential, if he had studied at Harvard he would have had the same Honors, and he has a nice personality...I want him on board. Apply per the suggestion FA. Be realistic and at the same time Bold However I think if u have good scores, excellent American letters. handson experience in the USA and research...apply to the top programs they will invite u. Go to the Interview and convince them why u are better
