I have a ? about diverticulitis vs. diverticulosis. which is more likely to bleed?

I can't help, but I doubt they ask that on a usmle exam.

bleeding diverticula are caused by a small blood vessel in a diverticulum that weakens and finally bursts this is more common in diverticulitis as infection weakens the blood vessel .

Diverticuli are tiny "pockets" of weakness in the wall of the large intestine. This condition is called diverticulosis, and a third of the people over age 50 in the United States have it. Some have just a few diverticuli, while others may have 20 or more. Most people have no symptoms and only find out that they have diverticulosis when they have a colonoscopy.

Diverticulitis is infection/inflammation of the diverticuli. In other words, a person with diverticulosis may get diverticulitis when stool gets caught in one or more of these “pockets,” and they become inflamed or infected. This typically causes fever, pain and tenderness in the abdomen. The two major signs of diverticular disease are bleeding and infection.

i can't remember reading it anywhere but i'm 100% sure that the MD who gave us a conference about diverticulitis vs diverticulosis told us that diverticulosis bleeds while diverticulitis does not.
i remember coz we had the same question a week later on an exam.

Bleeding may occur from the diverticulum in 5% of patients. It is usually sudden in onset, painless and substantial. Diverticulosis is the cause in 30 to 50 percent of cases with massive bleeding from the colon. However, the bleeding stops spontaneously in most patients. In some cases, the bleeding may continue intermittently for a few hours to a few days before resolving. Chronic intermittent bleeding due to diverticulosis is unusual. Blood transfusions are required in less than 5% of these cases. Some people believe that the diverticula bleed when they are inflamed. Actually, it is quite the opposite. Bleeding is usually not seen during an acute episode of acute diverticulitis. Diverticula bleed when they are otherwise healthy.

According to BRS path bright red rectal bleeding is frequent with diverticulitis.

Diverticulosis= having many diverticuli in the colon
Diverticulitis= inflammation of the diverticuli

Guys-This is like beating a dead horse. (How many aspects can ya ask about diverticulosis??)(I mean its common but there is little pathophysiology to it!) A higher yield would be comparing the intricacies of Crohns to U.C, or even gastric to duodenal ulcer or anatomical detail.

Goljan had as one of his practice problems something about diverticulitis & diverticulosis in the sigmoid colon.

He said diverticulosis bleeds more there than diverticulitis because diverticulitis causes inflammation & scarring off the blood vessels so they dont bleed as much.
