What's the best time frame after Sept 1 to start applying for programs? I read somewhere that it's best to apply between Sept 1 - 15 as most of the programs start receiving and processing applications during that time and call for interviews based on those received applications. Is that reasonable to assume?

What if someone is going to start observarship in September and can only get LoR for it mid or late September? What's best to do in that case? Mention you've secured an observarship at certain institution in Experience tab of CV and send emails/call over the phone programs you're interested and let them know LoR is available for that observarship?

Thanks in advance.

Alright. I can only share my husband's experience, but he is an AMG. He did a rotation in MD Anderson Cancer Center in September and felt strongly about getting a reference there. He sent in his application in late Sep or early Oct, and still got > 10 interview invitations from top programs. So for AMGs, it might not be a problem. However it can be a completely different game for IMGs. guess it depends on who your last reference is and the policy of the programs.
