hi bbb... I am an IMG with pretty good scores ...but passed my cs on the second attempt.. was searching on the programs... most of the programs specify step 1 and step 2 passing scores on the first attempt... only some specify cs first time pass as such.... should i presume the former do not mind a cs attempt...

Yes, if they are specific about Step 1 and CK, then they would have mentioned that for CS as well. But , the best way to do that is to contact (call/mail) the programs and clear this doubt. How strongly do you feel in your whole application and your scores? Is it worth the $ to apply? Would you really follow my advice if I said "don't apply" or "do apply"?
What is it worth for you?

What do you expect to learn from barbeque? Do you trust somebody that you don;t know? do you know if her/his answer are correct. I have heard so many stories that IMG went to good hospital with bad score because of research. People with excellent scores and research found one of the usual IMG friendly. I know couple of friends of mine that found very good university hospitals in big cities only with scores and with very few IVs.
Of course CS attempt is not good but it is not a disaster. Think about how a program director thinks. I mean if you applied to a university program with so many good applicants, why such a director invites you for interview. You must have something very attractive in your cv. eg research in the area of the program director or very high scores. Some of them may consider previous work experience as very important assets.
a friend of mine with a 2nd attempt had exactly the same interviews as another without 2nd attempt (that was in surgery).
No one can answer to you. You will never know
