- Hello docs, welcome!-

....This is another journal for me and for you too

First af all I want to say I was afraid to create my own journal because I don't feel as well prepared as all of you wise docs nod

....but I took the decision because I recently found that I need to feel "committed" to really do my best prep.......(just like Dr. Daugherty said: you need moderate "stress" to accomplish a purpose) nod

So here it is....and let me clear that these 7 things are probably not HY tested, or the difficult ones...maybe some of them very very basic for you, but tough for me....so I must remember them.....

I'll write daily what I've learned..........no matter what...........It's a deal..........(unless my computer crash, or I get booted of the internet sticking out tongue)..........obviously there are more than 7 learned things to remember......but I don't want to take up too much space.......

Hope this help me a lot with my prep...but also I hope you find this journal useful smiling face....

Feel free to correct me (also my english!!) if I wrote something wrong...ok!......That'll be very nice of you nod

P.S: Eligibility period: oct-dec 2008.....exam date not sure yet

Remember this:

1. (…..Just a correction to the past notes: ) Use clonidine for “ease” opiate withdrawal, use methadone for long-term maintenance.
In acute intoxication: naloxone (short 1/2life), naltrexone (longer ½ life).
2. Serum gamma glutamil transferase (GGT), sensitive indicator of alcohol use.
3. Caffeine psycopharmacology: antagonist of adenosine receptors, increased cAMP in neurons that have adenosine receptors.
4. PCP, Phencyclidine, Angel dust: similar action seen with ketamine
5. Some neuroleptics: thioridazine, haloperidol, fluphanazine, chlorpromazine
6. Some atypical antipsychotics: clozapine, olanzapine, risperidone.
7. Buspirone: does not cause sedation, or addiction. Does not intereact with alcohol.

Motivation for today:
YOU ALWAYS HAVE ENOUGH TIME IF YOU USE IT WISELY
Is what your doing getting you closer to your objectives?
Anything that is wasted effort represents wasted time.

Set priorities for your goals.
A major part of successful living lies in your ability to put first things first.
Most major goals are not achieved because people put second things first.

Don't serve time, make time serve you.

D' 7 things I've learned today....

(from Kaplan notes and UW)

1. SES (socioeconomic status): is a major factor influencing morbidity and mortality rates[/font]. Coronary artery diseases, stroke, lung cancer, and hypertension are experienced more frequently in persons of low SES. High SES is possitively correlated with breast cancer incidence.

2. Functions of SER (smooth end. ret.): 1) Detoxification reactions (steroid synthesis, hydroxylation reactions.C.P450, conjugation), 2) Glycogen degradation and gluconeogenesis (by glucose-6-phosphatase, and integral membrane protein), 3) Lipid methabolism (lipolysis begins in the SER. Release of a fatty acid from TG), 4) Secuestration and release of calcium Ions (sarcoplasmic reticulum = SER in striated muscle).

3. Some important intermediate filaments: Cytikeratins (epithelial), Desmin (smooth muscle, Z disks of skeletal and cardiac muscle), Vimentin (found in cells of mesenchymal origin: i.e. vascular smooth muscle), Neurofilaments, Glial fibrillary acidic protein (GFA).

4. Pemphigus: autoimmune blistering disorder caused by disrruption of desmosomes linking keratinocytes

5. Bullous Penphigoid: Autoimmune blistering disorder of the dermis-epidermis junction. IgG directed agaist an antigen in the lamina lucida.

6. Olfatory receptor cells: the only example in the adult human where neurons are replaced

7. KEEP GOING.....ACHIEVE TODAY'S GOALS...YOU CAN DO A LITTLE BIT MORE.....nod
