A mother brings her 10-year-old son to your office because he lost consciousness the day before while running. She says that he had had similar episodes in the past, but has never been seen by a doctor. She also states that he does not seem to be able to play as long as most of his friends. On physical examination you detect a systolic ejection murmur and find that the apical impulse is lateral to the mid clavicular line. Which one of the following is the most likely diagnosis?

1) mitral valve stenosis
2) aortic stenosis
3) coarctation of the aorta
4) aortic insufficiency
5) patent ductus arteriosus

Tx. The line about the apical impulse is a bit confusing. rolling eyes

I agree about the apical impulse. However, by exclusion, a systolic murmur can't be typically a PDA, AI, MS. A H/o syncope on exertion is one of the indication for severe AS.

2) AS
