What to mention under the Title column(DSF) if the letter writer is a private practitioner in the United states?

Private Practitioner? No title if they don't have an academic one? It's not a major thing

so title means the degree they hold?
What if they are an associate prof or something like that?
Do we put MD or associate prof

I would simply put the specialty (eg: "internal medicine" or "psychiatry"

If he is professor, you put MD and professor together + dept affiliation (say IM)...
It's meaningless to say Dr.XYZ, MD

Title means what comes before his name, based on his degree or academic rank,e.g Dr. Prof.,Mr etc.I stand to be corrected.
