A 60-year-old woman is undergoing surgical excision of a renal cell carcinoma thrombus that extends into the right renal vein. During the procedure, the vena cava is temporarily occluded above the renal veins. Increased venous pressure is most likely to be noted in which of the following veins during the period of occlusion?

A) External iliac
B) Hepatic
C) Inferior mesenteric
D) Portal
E) Splenic

it can only be between external iliac and the hepatic vein because those are the only two that are systemic
Inf. Mesenteric, portal, and splenic are all part of the portal system.

Now, I dont know if the answer is A or B. I am leaning towards A for the following reason.

I chose A because the pressure should increase in the external iliac vein because blood is being backed up due to the occlusion.

Not sure though, please help!

I think A too......just because of the procedure position!....

Its basically saying what drains into the IVC. Everything in the choices goes to IVC except IMV which is portal. The next part of the question ask which drains below renal vein because thats where thrombus likes and its the illiacs.

the answer is A
the only systemic tributary is the external iliac v
all the remaining form the portal vein which enter the liver then to central vein then to IVC this occur distal to the oclusion and doesnt affected
