Uworld says that increased pulmonary artery pressure will cause an accentuated P2 heart sound as well as paradoxical splitting. Can somebody please explain the paradoxical splitting in this case? I thought P2 would just get more delayed because of pulmonary hypertension or is it that pulmonary valve closes early because right ventricular pressure is not high enough to equal pulmonary artery pressure and so it closes early.

It is paradoxical because there will be no increase in P2 split interval on inspiration. The latter is physiological and occurs due to increaseRT ventricular filling time on inspiration
