The definition of Preterm labor is all of the following: *Pregnancy of 20-36 weeks *> 3 Uterine contractions in 3 min *Dilated > 2cm or changing (dilation or effacement) Kaplan says that once labor is confirmed with these 3 criteria and other contraindications to tocolysis have been ruled out, we have to give fluids and tocolysis. However it says that one of the Contraindications for Tocolysis is "advanced cervical dilation" Can anyone please explain this concept? Thanks.

If advanced cervical dilation (4 cm or more) or regular uterine contractions occur, likelihood of preserving pregnancy is small.

So what is the third criteria? Cervical dilation of up to 4cm ONLY? If >4cm (advanced) ---> NO Tocolysis (is that right?)
