While traveling by ship from India to Mexico, a 52-year-old man develops malaise, chills, and spiking temperatiures to 39.4C (103F). Over the next several days, he develops preogressive hypotension and muliorgan failure. He is taken to the hospital; a complete blood cell count shows a hemoglobin concentration of 5.0g/dL. A Wright-stained peripheral blood smear is provided (could not upload pic). Which is the most likely diagnosis? A) Leishmaniasis B) Malaria C) Meningococcemia D) Schistomiasis E) Trichinosis

Probably Plasmodium falciparum. This one causes rapid hemolysis of RBC's, hence the low hemoglobin. This is the most dangerous form of malaria and can kill the patient rapidly. http://en.wikipedia.org/wiki/Plasmodium_falciparu... http://cdc.gov/malaria/
