A 34 year old woman comes to the physicians office because she had a cough, fatigue, low-grade fever, mild dyspnea and aching joints for the past month. Physical examination shows no abnormalities. X ray of the chest shows bilateral hilar adenopathy with mild diffuse parenchyma infiltrates. Examination of tissue obtained by transbronchial biopsy during bronchoscopy shows noncaseating granulomas. Which of the following is the most appropriate therapy?

A. Azathioprine

B Methotrexate

C Prednisone

D Rifampin

E Zidovudine

My first impression was this is Sarcoidosis, but was suspicious, so I ran these symptoms in Epocrates -here is part of what came out -

Tuberculosis, Sarcoidosis, Lyme Disease, Hodgkin Disease, Primary Lung Malignancy, SLE, Pneumonia, Mononucleosis

Considering X ray findings and the noncaseating granulomas - i think this is Sarcoidosis.

Answer - Prednisone

Azathioprine and Methotrexate can also be used, but the first line of Rx is steroids.
