A 20-year-old man comes to the health center because of ankle pain. Two days ago he sustained an inversion injury of his left ankle in a basketball game. He has been able to walk unassisted since the injury. Today he has pain and moderate swelling and discoloration over the lateral malleolus. Physical examination shows tenderness on palpation over the anterolateral corner of the ankle joint. He has had two similar injuries in the past. Which of the following is the most appropriate initial management?


A ) An ankle-strengthening exercise program
B ) Application of a long-leg cast for 3 weeks
C ) Application of a short-leg cast for 3 weeks
D ) Protected weight bearing
E ) Surgical repair of the ankle ligaments

First-degree sprains or mild second-degree sprains
Rest, ice, and elevation
Compression dressing or commercially available air stirrup splint. Stirrup splints may result in better outcomes.
Consider initial cessation of weight bearing.
Early range of motion exercises
Consider referral to physical therapy for early range of motion exercise and wobble board training after recovery to reduce the number of recurrent injuries and to prevent functional instability.


Severe second- or third-degree sprains or possible fractures
Rest, ice, and elevation
Plaster or fiberglass posterior splint
Orthopedic or sports physician referral is indicated. Most patients require physical therapy to prevent functional loss.

the history is of a third degree ankle sprain. It requires initially RICE therapy and then CAST IMMOBILIZATION. for at least 3-6 weeks. short cast is used for ankle sprains.
for grade 1 ankle sprains, protected weight bearing can be used. there is swelling and discoloration (ecchymosis) in this injury. It requires IMMOBILIZATION.
ankle strenghtening exercises are performed after the cast is removed.

second reason for applying cast is there is Hx of RECURRENT INJURY
