which is better to do externship w/ an IMG who is practicing now here in States or do observership in a University affiliated hospital?plan is just 1 month in medical ICU, talked w/ the Medical director already told me that we could do rounds w/ them...VS the externship w/ a private IMG practicing here in US, also 1 month of externship...

I'd go for an externship at a private practice provided that the attending will let you do History and Physicals and this is to be mentioned by him in his LOR.
A simple observership many times is not counted by some programs as USCE. However, you should talk to the Medical Director and ask him for letting you participate in rounds, expose cases an have some participation in history taking and physicals as well. If the latter is posible,and all your participation is mentioned in your LOR, you might then describe this experience as an externship/observership. Thus this observerhip would be better than an externship in a private practice.

Also, can't you do 1 month externship at the prvate practice and 1 month observership at the hospital? That would be great.
