HERE ARE MY SUGGESTIONS
FIRST KNOCK KNOCK.. ..
NEXT ENTER WITH SMILE
NEXT HELLO MR. .......... OR MIZ(NEVER MISS OR MRS IN AMERICA)............ PT NAME.....
NEXT GOOD MORNING OR GOOD EVENING I AM DR. .............
NEXT I WILL BE TAKING CARE OF U TODAY,I WILL BE ASKING FEW QUESTIONS AND
THEN DO A BRIEF PHYSICAL AND THEN WE CAN DISCUSS ABOUT UR DIAGNOSIS AND TREATMENT OPTIONS, MEANWHILE IF U HAVE ANY QUESTIONS FEEL FREE TO ASK.
NEXT DRAPE THE PATIENT AND ASK WHAT MADE YOU COME TO THE HOSPITAL
I KNOW ITS BIT LENGTHY..........
I THINK IT WILL BE BETTER IF U PEOPLE SHARE UR WAY OF INTRODUCTION SO THAT WE CAN PICK THE ONE BEST SUITES US
PLEASE CONTRIBUTE IN DETAIL I FEEL THIS IS THE MOST IMPORTANT AND VERY GOOD OPPORTUNITY TO MAKE GOOD FIRST IMPRESSION AND IT IS SAME TO ALL PATIENTS

This is my way:
Write down Mr/Mz xxx, age, gender, vs, and cc.
At the right upper corner of blue paper, write down 3-4 DDs.
Write down LIQOR AAA and PAM HITS RUGS FOSS, and give enough space between them.
Knock, knock, 3 sec, then go in.
Hello, Mr/Mz xxx, I am Dr. xx. Shake hands.
I am the physician for you today, I will ask you a few questions and take physical exam. Is that OK for you?
Let me cover you up first.
CC: What causes you coming in today?
Can you tell me more about that?
Ask questions from LIQOR AAA, then ask a few questions from DDs.
PAM HITS RUGS FOSS Qs.
PE
Close up: include summary, explain diagnosis and test order, counselling, ask patients if they have Qs, assure patient, shake hands, say 'Take care.", then leave the room.

I learned this mnemonic for Grazie.
PAM HITS RUGS FOSS.
HITS stands for Hospitalization, Trauma, Surgery. For I, I think it is Illness, but maybe Immunization is better.
RUGS stands for Review of GU, GI and Sleep.

hi anneGermany, i think everybody should suitably modify PAMHUGS FOSS

and 1 2 3. 1 FOR URINATION 2 FOR BOWEL MOVT. 3.FOR MENSTRUAL HISTORY IN FEMALES

The order of PAM..... doesn't matter.

Introduction can be personalized. you don't have to strictly follow someone else's style. do what is comfortable to you.

ultimately whats important is - you should have clear mind and thoughts flowing in front of the SP. avoid blanking out. so make life as simple as possible.

location, intensitiy, quality, onset, radiation, alleviating, aggravating factors,
associated symptoms
PMH, allergies, meds, heart & lungs, urinary, GI, sleep, family history, ob/gyn, social history, sexual history
you can modify these however you want.
also look for the mnemonics thread.

H can also be "hospitalization"smiling face

Just a tip: when you introduce yourself, say "nice to meet you", wait for the answer. Give the patient a few seconds to get used to your face before going "what brought you here", etc. You enter with a smile, but if the patient is in pain, undo it right away! If he's in a bad mood, ignore it, keep the smile for a little while.

everybody seems so keen to ask what brings you here,i feel why to bug this already tired SP with same question over and over,so i suggest saying- i have learned from doorway info.....,is that your prob....etc so i mean cut the monotony, make SP INTERESTED IN YOU, i am thinking on those lines, anybody's comments for this ideanod i'd love to read,Thanks

  #13

I think for daily practice it's OK, but for this test, you HAVE TO start the encounter with an open question, not a leading one. I think you have to forget the SP is doing this over and over again, and pretend he's a real patient and you are the only doctor seeing him. You don't gain points for being original... wink Of course, you'll bring your personal spice to it, but don't think too much of it.
