It depends if the IV hyperlipoproteinemia is primary or secondary.Since Alcohol can cause secondary ,so in this case alcohol abstinence.If primary then Fibrates are preferred.

type 4 increase VLDL and TG so you give fibrates
type 2a increase LDL and Cholest you give statins
