A 52-yo F is evaluated for DM and CAD. Her BP 124/72 with diet alone. For her DM she uses metformin and glyburide. She is nonsmoker.

What is target value for her LDL?
<70
<100
<130

Depends, if she has had an acute coronary syndrome or diabetes with peripheral neuropathy she will straight away go in the <70 bracket.I did'nt get it from the qs whether she has known CAD.If she has CAD goal is <70.If she does not have CAD,then DM(if it is uncomplicated) will cause her to fall in the second group----> Goal LDL<100.Actually depends on HDL levels whether they are raised.

Here she has only one risk factor i.e DM ,so < 130 is ok .

she has both CAD and DM.
the target for pt with history of CAD and and another major risk factor is and LDL <70 (regardless of HDL)

i thought that it is CAD and its equivalent target <100, but it appears that if combo - <70, if CAD or its equivalent - <100

p.s. it's Fischer's question

CAD even alone means target for LDL is < 100 .But stem question says that she is evaluated for CAD but does not mention if she has CAD. Is this not so ??

Here comes the idiotic question of the day from me

If they do not have disease less than 130 LDL
If they have disease less than 100 LDL

If patient with no disease has more than 130 LDL you start with simvastatina.
If patient has disease with more than 100 you start simvastatina

CORRECT?

If <100 mg/dl: instruction on diet and exercise, and repeat annually
