A 35-year-old man who is planning to go on Safari in East Africa for 4 weeks comes to the physician for prophylaxis against malaria. Which of the following drugs is most appropriate?

A) Diethylcarbamazine
B) Ivermectin
C) Mefloquine
D) Praziquantel
E) Trimethoprim-sulfamethoxazole

I chose C – Mefloquine. It is an anti-malarian drug used for suppressive prophylaxis. Mefloquine is only effective at killing the malaria parasite once it has entered the erythrocytic stage (blood stage) of its life cycle, and therefore has no effect until the liver stage is complete. Therefore, the patient must continue to take the prophylactic drug for four weeks after leaving the area of risk. Side effects are severe depression, anxiety, paranoia, aggression, nightmares, insomnia, seizures, peripheral motor-sensory neuropathy, vestibular damage, central nervous system problems, and suicidal ideation.
