Some PDs wont bother to see if you waived or not. Some will definitely give more weightage to a waived one. The problem with waiving the right to view is that you wont be sure how good or bad the LoR is. moreover, you cannot have it back in case you need it for the next match season.
1.Yes, all LoRs need a cover sheet.
2.Which documents are you talking about? in any case getting them notarised wont harm.
3.No, waived LoRs will not be returned
4.Send the new LoRs just like you send all your documents. With a cover sheet for each LoR and a DSF.

I am not sure, but i dont think they need to be notarised. i think i read it in a thread recently that people have sent copies without notaisation and had no prob. But like always, the most reliable would be the source itself. (ECFMG)
