If I want to apply for IM, only for prelim, and all my cv and recom show that I am interested in another specialty, how can I prepare a LOR appropriate for IM prelim? please help

Well if the other specialty you're interested in is an advanced program such as Anesthesia or Neurology then you wouldn't need to prepare anything else. It's understood that those applicants going for an advanced program need one year prelim.

thanks for answer. sorry if this is basic, but what do you mean by "advanced" program? you mean the specialty I want to continue after the prelim? is it all the specialties except IM?

An advanced program requires a prelim IM year. Anesthesia and Neuro are advanced programs. See the thing is - if you want to get into IM then it's better you apply for a categorical position. PDs will want to know why you want to do a prelim year if you're not applying to an advanced program. On the other hand, if you're undecided as to what you want to get into, then you can look into applying to a transitional year residency (it's one year). Good luck.
