A state-mandated blood screening test conducted on day 2 of life in a healthy full-term newborn shows a decreased serum thyroxine (T4) concentration and a normal serum thyroid-stimulating hormone (TSH) concentration. Tests on venous blood confirm these findings. The venous free T4 concentration is normal. Which of the following is the most likely explanation of these findings?

A) Dysgenesis of the thyroid gland
B) Hypopituitarism
C) Inborn error of thyroxine synthesis
D) Iodine deficiency
E) Thyroxine-binding globulin deficiency

E. because total T4 is decreased, still free T4 is normal. so decreased total T4 must be due to deficiency of binding protien
