76 year old patient with history of hypertension and diabetes has dry skin , cold intolerance, constipation . BMI is 27 . Treatment is started with Thyroxine . The patient is at increased risk of developing which of the following during 1st month of therapy ?


A. Angina pectoris
B. Asthma
C. Exopthalmus
D. Goiter
E. Jaundice

a. angina pectoris; i remember being told in ck lectures that in elderly patients u always have to monitor their HR while taking thyroxine, bc they are sensitive to the changes and especially of the heart; the other choices dont make sense to be chosen... sorry it wasn't explianed better sad

I agree, it is Angina pectoris. This I believe is because Thyroxine increases myocardial oxygen demand. DM would already have caused a vasculopathy.

history of hypertension and diabetes .....so she most probably astherosclerotic

thyroxine will increases sensitivity of receptors to catecholamines

increase heart rate ........> increases oxygen demand..........>astherosclerotic coronaries........> angina
