i mean where we have to explain a lab result to a patient .. maybe a pathology report ; cancer

no we dont take history in the case of disclosing lab result. we just disclose result in 5 phases( 5 phases are given in kaplan course Hand out).
in PN u can write amount of information provided. for example if they give information about pts history then write it in history section. if they provide about physical examination then write it into physical exam section. leave rest of the PN blank
