I know you shouldn't put treatment or consults on the pt note, but what do you say if a pt has suicidal ideations? How do you document this on the patient note? Thanks!

u can say pt thinks about ending life and if he had any plans u can sya that actually if he has a PLAN then it is an indication for admitting.. u counsel the patient : make him agree that he will report to u or ER if he has any suicidal thoughts .. will u do that"? he should say yes ( this is called suicidal contract.) about actually admitting, depending on state rules.. it varies.. and only psychiatrists can certify the need for the patient to be detained in such cases atleast in some states.. so u need not mention that please correct me on that. an american grad should be in a better position to comment on that.
