A 31 yo woman complains of severe LBP after playing tennis for 4 hours. She begins taking Ibuprofen every 3 hrs but finds little relief. She is prescribed codein for her pain relief. Her physician also prescribes Misoprostol since the pt has a history of bleeding gastric ulcer 1 year earlier after taking Ibuprofen. She has also been takig Sumitriptan for frequent migriane. Which of the following is the most appropriate advice for this pt?
A. Avoid dietary K+
B. Increase dietary roughage
C. Use an effective method of contraceptive
D.Increase her daily folate intake
E. Increase her daily aerobic exercise acitivity

patient on misoprostol shouldnt get pregnant because prostaglandins cause abortion.. if she wants to get pregnant, misoprostol must be stopped.. here, she must use contraception to avoid unnecessary abortions

Dear Anand007,

its true that it causes abortions but whats the use of contraception then? doesnt it cause also prevent pregnancy? I know this is what is written in kaplan for step 1 but still, it doesnt make sense smiling face

or does it? if yes then please explain...

There is recent data regarding misoprostol to cause Mobius syndrome (congential facial paralysis) plus there is suspicion regarding teratogenesis of this drug ...

we give good contraception cuz we dont want her to get pregnant in the first place so she wont get an abortion with misoprostol....it makes sence,,,what doesnt make sence to u waqas????plz explain....

Thanks for nice explanation Anand007,

But it seems that many kinds of Prostaglandine including one kind applied for uterine contraction. The problem is misoprostol applied for which prostaglandine?

Another thing Codeine causes constipation. Why not B?
