A 56 year old woman has shortened thickened chordae tendinae and a stenotic mitral valve on autopsy. what is the most likely cause?

a. group a strep.
b. coxsackie b
c. strep viridians
d. treponema pallidium
e. s aureus

i cant decide b/w a or b. a little help please.

it is due to rhumatic fever which is the most common cause of valve disease especally mitral valve.
coxacke affect young adults without any predisposing factor

It should be rheumatic.. But not 'rheumatic fever'. It's 'Rheumatic heart disease' . theres a lot of difference between 'carditis of ARF' and 'RHD', esp from the t/t point of view. The answer would be a. group a strep. in any case.
Besides, doesnt coxsachie B virus cause myocarditis and pericarditis. Does it even cause endocarditis/valvular defects?

Coxsackie causes myocarditis & tuberculosis causes pericarditis.

Although myocarditis is more common with coxsachie B virus, it can cause pericarditis as well
