1) CAUSES OF SECONDARY HTN------conn syndrome(primary aldosteronism) , postductal coarctation( activation of RAA) , pheochromocytoma and neuroblastoma( increased cathecholamines), OCP's ( increased angiotensinogen), cocaine(increased sym. ) graves dz( systolic Htn) , renovascular dz in men due to atherosclerosis and in women due to fibromuscular hyperplasia.

2) MC type of HTN----essential HTN ( decreased RAA due to increased plasma volume which is due to reduces sodium excretion)

3) renovascular HTN------increased RAA cuz of decreased blood flowing to the kidneys and uninvolved kidney has decreased RAA...(I hp it makes sense)

4) Takayasu's and giant cell arteritis----large vessel vasculitis and granulomatous inflamm.

5) kawasaki dz----kids, desq. rash, MI, corticosteroids are contrindicated as danger of vessel rupture

6) polyarteritis nodosa----spares pulm. artery and ass. with HBsAG

7) churg strauss syndrome----allergic rhinitis, asthma, P-ANCA abs.

1) LHF ----forward failure , dyspnea, pulmonary edema, mitral valve regurg. can be seen and paraoxy. nocturnal dyspnea is an eg.

2) RHF----backward failure, increase in venous hydrostatic press., distended jugular veins, painful hepatomegaly and ascites and depedant pitting edema.

3) high output failure-----vasodilation of arterioles, seen in endotoxic shock and thiamine def. and in severe anemia.

4) stable angina---ST seg. depression (subendocardial ischemia)

5) prinzmetals angina---ST seg. elevation(tranmural ischemia)

6) ECG in MI----inverted T waves, elevated ST seg. and Q waves

7) S3 heart sound----first cardiac sign of LHF.

hi match, how is your studies going? are u watching videos or just going through kap bio?

@drdedication, i dont want to intrude here, but Goljan is for patho, and there are videos for other subjects, you can buy from ebay or other classified sections( if this one doesnt, probably usmlenet, or usmleforum will have classified)

thnx pursuit, KG , AD and killer for stopping by ...and drdedication i think AD answered ur Question.

1) causes of tissue hypoxia--- ischemia , hypoxemia, left shift curve, methb, CO poisoning, anemia,uncouplers .

2) O2 saturation ----is the oxygen sitting on the hb.

3) Pao2----is the oxygen dissolved in the plasma

4) decreased oxygen saturation and normal Pao2-----seeen in CO poisoning and methb.

5) TX of methb ----IV methylene blue and ancillary tx is vitamin C (reducing agent)

6) NITRO AND SULFA drugs----does methhb and has potential for producing hemolytic anemia in G6PD def.

7) 2,3 BPG---binds to beta subunit of HBA and causes right shift of the curve that is decreased affinity for oxygen so tht it can be given off to tissues

8) CALCIUM --- produces irrevesible damage to cell , enters mitochondria and release cytochrome C.

9) acetaminophen---MCC of drug induced fulminant hepatitis as it produces Free radicals which can be neutralized by glutathione.

10) alcohal----MCC of fatty change in the liver.

11) glycerol-3-phosphate----its made in glycolytic cycle and its a substrate for TG's synthesis .

12) coagulation necrosis----preservation of structural outlines , lactic acid accumulation due to denaturation of proteins(infarctions)

13) liquefactive necrosis----cerebral infarctions and cerebral absecess comes under this and produces a cystic change in the brain which involved neutrohils.

14) caseous necrosis----TB organisms and all systemic fungi's.

15) enzymatic fat necrosis-----unique to pancreas only.

16) bax gene---apoptosis gene

17) bcl2 gene---antiapototic gene

in a bad place really... but the only way out of this mess is through it...
