Yeah this is how i feel about the step 1 smiling face Time is running and i need to take control of time and not let it haunt me, Im coming out of the habit of saying ''tomorrow'' and putting my step with the word ''today and right now''...TODAY IS THE FIRST DAY FOR THE REST OF YOUR LIFE ...so I have already strted studying hard but i will push it now to 13-14 hrs daily actively. God please give me the endurance and potential to stay determined and sincere on the path i chose and give me strength to study actively 13 -14 hrs daily till the end of my exam .

TARGET= 240+ ( so i need to work very very hard to acheive this in short period of time)

Everyone is writing or making FA thread or Kaplan notes thread ...so from today im Starting a goljan thread ....i will write 7 points daily from goljan in my journal which i think is important.

alrite guys too much motivation now ..lets get back to books and will report tonite ....

good luck studying .....

ALRIGHT ...so from today as i said i will start with 7 point daily from rapid review so anyone who doesnt have time to read it can look at this daily ........(these points are only from goljan )

1) BLACK WIDOW---painful bite, neurotoxin

BROWN RECLUSE SPIDER--- painless, very dangerous and its necrotoxin

2) All superficial dermatophyte infections are due to TRICHOPHYTON RUBRA except Tinea capitis which is by TRICHOPHYTON TONSURANS.

3) A child comes who is hit by baseball in the eye and started dripping a fluid from nose ? How would you tell if its CSF or just fluid ? ( Know that CSF has increased chloride than serum and fracture was in cribiform plate so thus leakage of CSF from nose )

4) EPIDURAL hematoma----Btw bone and dura , rupture of middle meningeal artery

5) CSF flows---- produced by choroid plexus in the ventricles and goes from 1,2----3--aqueduct of sylvius--4th ventricle---foramen of magendie and luschka----SA space--reabsorbed by arachnoid granulations---into dural venous sinus---jugular vein----R side of the heart.

6) VALSALVA MANUEVER---- holding breath distends neck veins and thus back buildup of pressure all the way through spinal fluid and SA Space and thus you can tell that entire SA space is patent when doing spinal tap.

7) OCULOMOTOR PALSY----- eye is down and out , and can be seen in uncal herniation.

TROCHLEAR NREVE PALSY---- eyes are down and in.

1) A pt. with head trauma is purposely hyperventilated---- SO tht we can produce resp. alkalosis by blowing off more CO2 and thus causing cerebral vessel constriction (remember decreased CO2--> decreased blood flow to the brain) so we decrease the risk of cerebral edema.

2) Neural tube defects --- increased alpha fetoprotein ( down's have decre. alpha fetoprotein)

3) Tuberous sclerosis ---> ass. with ash leaf lesions on skin, angiomyolipomas, rhabdomyomas, hamartomas in brain .

4) Neurofibromatosis---> ass. with lisch nodules, phechromocytoma, acoustic neuromas

5) MENINGITIS IN NEONATES---- MCC is gp. B strp , 2nd MCC is E.coli

6) pregnant women sudnt eat soft cheese in pregnancy----> L. monocytogenes related neonatal meningitis

7) N. meningitis---> MCC of meningitis from 1 month old to 18 yrs old

strep. pneumo---> meningitis >18 yrs. old ppl.

1) Rubeola(measles) --- RNA paramysovirus, koplik spots on buccal mucosa and then maculopapular rash.

2) Rubella(german measles) --painful postaruicular lymphadenopathy , 3 day measles, maculopapular rash lasts 3 days.

3) common cause of febrile convulsions---roseola

4) parvovirus b19-----aplastic anemia and pure red blood cell aplasia, slapped cheek .

5) Leprosy--- 2 types( milder is tuberculoid with intact immunity and dangerous form is lapromatous with no granulomas and lack of cellular immunity)

6) androgen receptors on skin? ---present on sebaceous gland thats why men get more acne, and block androgen recpetor by spirinolactone (blocks androgen and can cause gynecomastia)

7) dandy walker syndrome---- cerebellar vermis hypoplasia
