JOTHI,expectant mother with 20 weeks pregnancy has come for evaluation. physical examinaton revealed HB 12G/dL ,HR:103/mt,BP 130/90 mm of Hg, and temperature 98.6 F. She gives the histrory of diabetes for which she is taking oral hypoglycemic drugs and it is in good controll. what is the most appropriate method of investigation to find out malformations in foetus?
a.ultrasound
b.MS-AFP
C.FOETAL ECHO
D.chorinoic villous bipsy
e.PUBS
F.FOCUSSED ULTRASOUND

it must be usg.at 20weeks with usg we can find malformation,cvs is done at 12week,pubs at 20weeks it is invasive and pregnancy loss is 3-5%
usg is noninvasive and safe

PUBS...percutaneous umblical blood sampling..done after 20wks for fetal karyotyping,IgM Ab detection,blood typing and intrauterine bld transfusion.pregnancy loss is 1-2%
i think its USG....most common fetal malformation in uncontrolled DM is cns and cvs..

i was just read this and thought i should share it with u...
if the mom is overt diabetic then we follow her up antenatally as..
1.home glucose monitoring
2.glycosylated Hb
3.renal and retinal status
4.15-18wks....triple marker screening
5.18-20wks....targeted USG
6.22-24wks....fetal echo
7.mthly USG for macrosomia and IUGR
8.wkly NST ...after 32wks

so i guess the answer should be targeted USG as this patient is 20wks...
