A 55 yr old man develops SOB. Examination show harsh systolic murmor at the base that radiates to the neck. EKG shows evidence of prominent left ventricular hyperthophy. Which of the following cardiac abnormalities is most likely? a) mitral stenosis b) mitral insuficiency c) bicuspid aortic valve d) aortic dissection e) tricuspid insuf. Any help is apreciated. Thanks!

answer B, Harsh sysolic murmur radiating 2 neck+LVH-MITRAL INSUFFICIENCY.In MR,the blood regurgitating during systole from LV to LA comes back during diastole causin LVH. In MS,A DIASTOLIC murmur is heard,In bicuspid aortic valve if calcifies leads to AS-ESM at the right 2nd intercostal place,AR-WE HEAR A DIASTOLIC MURMUR,In TR-a sysolic murmur is heard over the right side and they may say the murmur increases on inspiration,all right sided murmurs increase on inspiration,there is no LVH,RVH is seen in TR.

LV hyprtrophy - AS, in MR - LV and LA hypertrophy While dispnea is seen in MR, AS would be angina and syncope.

am with C this is classic presentation of aortoc stenosis(systolic murmur adiate to the nech) bi-cuspid valve lead to increased probability of calcification earlier age than normal aortic calcification that occur with aging

In MR,LVH and LA enlargement not hypertrophy seen,systolic murmur at the apex radiating to axilla n left sternal border.In AS,CRESENDO-DECRESCENDO MURMUR best heard at right 2nd intercostal place radiating to carotids,LVH.In the question,the murmur is systolic best heard at the base of heart radiating to neck,does aortic murmur be heard at the base of heart?

Yes, aortic murmur at the base, mitral at the apice. Remember the heart is upside down. The answer is C.
