Following is what I found written for pronator drift, drift in general and cerebellar drift....supinators remain weaker

Testing for pronator drift
With an UMN lesion there is pronation and downward drift of the outstretched supinated arm. This is because the pronators overpower the weaker supinators. Another name for this sign is a pronator Babinski.

Drifting is the movement of one or both arms from an initially neutral position. It is the result of upper motor neurone, pyramidal lesions.

There are three causes of the weakness causing drifting:

pronator drift
cerebellar disease
posterior column loss

To test for cerebellar drifting, the patient is asked to hold both arms fully extended at shoulder level in front of him, with the palms upwards.

If cerebellar drift is present then this is secondary to hypotonia and the drift is usually upwards.

Thank you so much mukho

this is what i understood-

umn lesion- downward pronator drift

cerebellar-upward drift

post columns-psuedochoriathetoid drift

yes pronator drift is due to UMN problem

there is difference of tones between flexor and extensor as welll as pronators and supinators. Embryologically, pronators are like extensors. In UMN, there is more EXTENSOR response, so even a small increase in tone can be noted by drifting of hands into pronation.

I hope others have explained how is it done. This is quite sensitive for UMN and is being used in various screening tests for evolving STROKES
