I've been suffering from the form 186 syndrome. It's anxiety with palpitations when I hear the number 186! I don't know if anybody had a problem with form 186 and I hope somebody can help. My story: I recently registered for the exam (the online part) and before ending the registration process I was asked to download form 186 and print it out. After I printed the certificate I had to fill it up and get it signed by a notary. So I went to the notary who refused to sign it because the document is in English. I'm from a small Arab country called Lebanon. And apparently public notaries here do not legalize any paper not in Arabic ! Later I called the prometric center here in Lebanon (AMIDEAST) hoping I'll get a good guidance about this issue only to find out that they've never heard about form 186 before ! Had anyone had a problem with form 186 before. If no notary can sign it who can ???? Please help cause I'm about to collapse on this issue !

I do not know the answer to your question. But I think calling the ECFMG and talking to some people from your med school or from your country, who have already been through the process will help. Good Luck!

Why will ur medical school not sign it? About notarisation, u could also call the US consulate and see if they will agree to notarise it for u (they do notarise copies of original documents for a set price, but am not sure if they will notarise am identification form like this, but u could try) and as hitesh said best to ask people from ur school who have done it before
