A 22 y/o man used illicit IV drugs briefly 1 year ago. He is asymptomatic and has no history of viral hepatitis. Serum ALT and bilirubin concentrations are normal. Serologic studies show:
Hepatitis B surface antigen (HBsAg) positive
Hepatitis B core antibody (HBcAb) positive
Hepatitis B e antigen (HBeAg) negative
Antibody to surface antigen (Anti-HBs) negative
Antibody to e antigen (Anti-HBe) positive

Which of the following statements best describes the current condition?

a-He is in the incubation period, is highly infectious, is likely to develop acute hepatitis B.

b-He is in the incubation period, is minimally infetious, and is likely to develop acute hepatitis B.

c-He is a chronic carrier of hepatitis B and is highly infectious.

d-He was infected previously with hepatitis B. has recovered and is immune to hepatitis B.

i feel he is chronic carrier but not highly infectious..cos surface antigen positive and also anti Hb e positive...wat say ppl??

Antibody to surface antigen (Anti-HBs) negative = no immunity

Hepatitis B e antigen (HBeAg) negative = not highly infectious
Antibody to e antigen (Anti-HBe) positive = ??
Hepatitis B core antibody (HBcAb) positive =immunitysmiling face

There must be an error somewhere in the question, i dont know i'm going with B because i think hes infected but minimaly infectious....does that even make sense:S

I wouldn't say that he recovered since he is HBs+, so he has an infection either acute or chronic (if he had recovered from the illness, he wouldn't have HBs Ag).

He can't be in incubation period since he managed to produce antiHBe antibodies which appear after disappearing of HBe antigen.

Presence of antibodies to HBc Ag can be either in acute (IgM) or chronic infection (IgG) or after resolution as a sign of healed illness (IgG).

So, I am not sure between B and D, neither of them satisfies me completely

HBcAb-IgM (+)------>acute hepatitis
HBcAb-IgG (+)------->chronic or late acute hepatitis.
HBeAg (-)-------->minimal infectious.

the only difference between the chronic active hepatitis and chronic carrier state is the HBeAg. if infection is there, as shown by the HBsAg positivity and HBeAg is negative, it is carrier state...the correct answer is

ok, it can be C - in the sence that it is chronic carrier, but why and is he highly infectious when he is HBe negative (since it is the part of the answer C)?

Among the choices B might be the answer but should be something like " previously affected, chronic carrier and viral load is low so not infectious"

I think it can't be B since he can't be in incubation since he has already made antiHBe antibodies which develop after HBe Ag dissapear

I don't think he should be 'HIGHLY' infectious.

Pre-core mutant is a hepatitis B virus with mutation in the precore promotor region. This can occur in the process of pathogenesis or might have taken place before infection. A precore mutant doesn't carry HBeAg. So the rule of chornic Carrier eAg negative, doesn't apply here. Besides, precore mutants are more resistant, requiring longer duration of treatment with interferons and are more liable to cause cirrhosis.smiling face
