how to test for thyroid? just ask the pt. to swallow simply or offer him a glass of water and then ask him to swallow?

This is how I do it:
"I will step behind you and place my hands around your neck to feel for your thyroid gland. Is that ok with you?"
If patient asks what this is:
"It is a gland in your neck that produces hormones"
Then you place your hands around the neck as if you wanted to choke the patient and feel for the thyroid with your digits II.-V.
Plapate for nodes, swelling etc. and then ask the patient to swallow. Feel if the thyroid moves with the swallowing.
That´s how I do it, maybe somebody has other suggestions.
