hi after listening kaplan video and looking kaplan pharmacology i m confused that what should give first in organophoshprous poisoning , atropine or pam?according to video atropine first , according to lec notes pam APSP

No No No For symptoms like Mucle twitching, diarrhea, lacrimation, salivation appears, then give Paralidoxime Atropine to keep HR above 60 ( to avoid bradycardia). Its not that you should give which one first Depending on Symptoms.

i think u must give pralidoxime assoon as possible because they compete with organophosphate for phosphorus receptor so, if you don't give it asap , it will bind irreversibly with AChEIs

Yes, there's a window where organophosphates bind reversibly to AChE and before an irreversible complex is formed. PAM needs to be given asap 'cos it helps only till this binding is reversible Its true that atropine is for symptomatic management and support.
