This is a good question, I will try to find more

Case with Confabulation.-

62 year old white male had been a known heavy alcoholic spree drinker for many years. Two years previously he was admitted to the hospital because of delirium tremens. After his most recent drinking spree he found himself in Florida (he lived in Boston), not knowing where he was. Apparently he had drifted aimlessly for five weeks with no definite food intake for a month. Neurological examination revealed diplopia, nystagmus, ataxia, marked memory impairment, Confabulation, disorientation for time and place, with no insight for his disorientation or the reason for his hospitalization, and decreased sensation in distal lower extremities.

Clinical Diagnosis: Wernicke-Korsakoff syndrome

The patient was treated with Thiamine. The diplopia disappeared and there was marked improvement in extraocular muscle function.



Whenever someone has a possible diagnosis of alcoholism, and then has the sudden onset of memory difficulties, it is important to seriously consider the diagnosis of Korsakoff's syndrome.

While there is no specific Laboratory test to diagnose Korsakoff's syndrome in a patient, a careful exam of the individual's Mental State should be rather revealing.
Although the patient's ability to confabulate answers may be convincing, checking the patient's retention of factual information (asking, for example, for the name of the current president of the United States), along with the patient's ability to learn new information (repeating a series of numbers, or recalling the names of three objects ten minutes after having been asked to memorize them) should point to the diagnosis. Certainly a patient known to have just begun recovery from Wernicke's syndrome, who then begins displaying memory difficulties, would be very likely to have developed Korsakoff's syndrome.
A Physical Examination may also show signs of Wernicke's syndrome, such as peripheral neuropathy.

Don't sit, promise to be quick, say the nurse is going to bring medication, say there is (or there isn't it) a chance a surgery may be needed, show empathy and eficiency.

And remember that this is not a real life situation, so there is no real emergency. Just keep telling the SP that you understand what he/she is going through and you'll get him some medication ASAP... But, take your own sweet time to take a good history. ( No need to pop a nitrate pill in a case of chest pain) wink Dont let your real-doctor-instincts come in the way of your doing well at the exam.. grin

dont worry about confabulation dear Paheli smiling face Im back , and trust me , this exam is nothing like that !! Its the horses again !! and even if you suspect that , it would cause you to lose 1 credit out of like 800 smiling face

its all the main most important cases in FA and no complicated , 2 in 1s ! and the SPs ar really helping ! I dont know where the rumors about Philly come from smiling face

If the confabulation is a plausible history, the only way for you to know it's fictional is when the family tells you nothing like that happened. So I don't think it is going to happen in this test.

And since I am talking about this, when checking for memory, always ask things you are able to know are true or not. You know how the weather was yesterday, what happened in September 11, 2001, but you don't know in what year she got married, so don't use that.

I actually did it on a vertigo patient i had...there isnt much else to go in vertigo patients I thought...u dont be clinically perfect in eliciting results but doing it clearly enough to make the SP realised what u did sounded like a good idea to me...didnt take too much time either...u should decide on basis of time maybe
