I will not be able to provide "The Dean's Letter" or MSPE for ERAS. The reason is that the format of this letter is so specifically fitting US medical training approach but so different from what is used back home. No official is going to spend all this time trying to figure out instructions and struggling with the frustration of adopting the form to local realities. Although I graduated at the top of my class and this letter would be very useful for me and, yes, "graphical" as they expect it to be, instead I am going to ask my Dean to write a good letter of recommendation where to mention that the official MSPE form could not be meaningfully used.
AM I MAKING A MISTAKE?
Anyone got a similar experience????? (doing w/o Dean's letter, i.e. MSPE)

I have the exact same doubt, and though I donīt have an answer for it. I'll post this reply with a hint that someone gave me, hoping that by moving this post to the top of the list someone more informed will enlighten us.

The ECFMG sends a notice of unavailability for the "Deanīs letter". I guess that PDs already know that IMGs cannot provide MSPEs, so they will not be looking for it. Someone told me that when you are an IMG, USMLE scores and letters of recommendations are all that matters.

I know a lot of people who got in to really decent residencies without having ever submited anything signed by the Dean (exept of course for the steps application forms).

Anyway, I guess my answer is not very useful.

PD's know about the difficulty in getting some dean's letter. As long as you have some really strong letters - and it sounds like you have that taken care of, you should be good to go.

How strong can a LOR get when they do not come from people who practice medicine in the US? I could easily be recomended by faculty from my med school, even by the dean, but is that "strong" in any way??

I know a few doctors currently practicing medicine in the US and they will be happy to recommend me, but they can only talk of my personality, and my happy predisposition to work hard, coz in fact, they have never see me work. Is that any good?

I am in a tough crossroads here... i have really good scores (99 in step 1, 99 in step 2) but absolutely no way to find "strong" letters from american doctors, let`s not say american faculty. What could be get me around this problem?

So do I submit the free-style letter in lieu of MSPE?
That is, if I understand you correctly, I should not choose "I will NOT be able to provide Dean's letter" option and thus not opt for the "automatic Dean's letter" ERAS creates for you.
Rather I should mark "I will be able to provide Dean's letter" and submit my free-style letter even though the format, etc. is very different? Just use what I've got? Is it possible to do so? Or do I submit my improvised letter as one of the LoRs?
