Can anyone offer me some help in the following 2 questions? 1. A 25-year-old motorcyclist was brought in after a traffic accident. He had an open fracture over the femur and blood had been pouring out. His airway and breathing had been attended at the ambulance. You are the A&E house officer, what is the most important action at this point? Should I say blood transfusion, or wound bandage, or IV access, or obtaining consent for OT? 2. You are the house officer at the medical ward. While you were taking blood from a 40-year-old woman with anemia, she suddenly said that she had a "funny turn" and then collapsed. You immediately stopped the blood taking and disposed of the needle. What is the most important action next? Would it be IV access? Thank you very much!

It always goes as ABC...so for the first one if Airway and breathing are dealt with the next step is circulation...OT consent????? impossible throw it out of the window..blood transfusion not possible without an iv access.... only yes bandage or iv acces..open fracture...immobilse but bandaging at site or in transport is out of question.. so my dear fren "hir" is right...it has to be iv access

You did not give options for next one... it really cannot be judged without it...such fainting episodes are usually due to sudden decrease in blood flow to the brain.. and just making the patient lie down and elevating the lower limbs immediately rescues the patient.. but yes i dunno about the options... but for refractory cases iv access is the next step of action

For the second case, she collapsed, you go like this: call for help, check airway patency, thrush the jaw if needed, check breathing and pulse, initiate CPR if needed, defibrilate as soon as you have the machine. IV would be after all this.
