what is the rx of lone AF?

only Anticoagulants(asprin)this is what i have written in my notes dont remember wether it is from q bank/UW. is this right?

unstable and acute stable cardioversion
chronic >48 hs. rate control (diltiazen metaprolol ,digoxine) + without riesk factor aspirin, or pte. with riesk factor warfarin.
the riesk factor are =1 stroke or TIA history
2>65 y.o
3 diabetes or hypertension
4 heart failure
5 valvulopathy

AF Hemodynamically Unstable:
Cardioversion
a)Electrical 100-200 Joules
b)Chemical:Type III Antiarrythmics e.g Sotalol,Amiodarone.
If <48 hrs=Acute
If >48hrs =Chronic =use anticoagulant before cardioversion for a couple of weeks

AF Hemodynamically stable
Rate control: Verapamil/dilitiazem,B-Blockers,Digoxin(Associated with CHF)
Cardioversion
If <48 hrs=Acute

If >48hrs =Chronic =use anticoagulant for a couple of weeks + Rate control as above

Urgent rate control can be accomplished with a beta-blocker, calcium-channel blockers, or digoxin. Digoxin is relatively contraindicated because of the renal insufficiency. In addition, rate control with digoxin takes much more time than using metoprolol. Even intravenously, digoxin can take 6 to 12 hours to be fully effective.
Amiodarone is best used in patients with ventricular dysfunction.

Lone AF have a lower risk than precipating-related AF (of mitral regurgitaion/stenosis...) for coagulation. Aspirrine is appropriate, especially young patient.

Lone AF-

No h/o of DM, HTN, Heart Disease, Coagulopaty,l ess than 65 yrs of age-ASPRIN -no need of rate control

with any of the above conditions -warfarin + rate control
