A 39 yr old XY is known to have been infected with HIV for the past 8 years. Despite antiretroviral therapy,his HIV -1 RNA level has recently been rising and now is 28 000 copies/ml. He s found on PE to have stool positive for occult blood. Sigmoidoscopy reveals a 4 cm mucosal mass in the upper rectum. Which of the foolowing conditions is most likely to be associated with these findings?

a) ALL

b) AML

c) CLL

d) Diffuse large B cell lymphoma

e) Follicular lymphoma

f) Hodgkin disease, mixed cellularity type

d) Diffuse large B cell lymphoma << This one was an easy one but i dont know why it took me sure a lot of time to figure it out.

EXPLANATION

Non-Hodgkin lymphomas of the large diffuse B cell type can be associated with HIV infection and define the AIDS. AIDS lymphomas tend to be extranodal and the large cell type can also involve CNS; they have a poor prognosis. In approximately 50% of cases the systemic diffuse large lymphomas contain the EBV genome.

Virtually 100% of AIDS associated CNS lymphomas are infected with EBV.
