does any1 know the format for patient notes in a prenatal visit case?

wl they keep a pregnant woman sp?

hi my question was somewhat similar to urs..
what do we have to write in differentials of a pt who comes for first prenatal visit?

i was wondering wat to write in the physical exam part ? like fundal height...etc
or do we write the usual abdo exam;like-soft..,NT,ND....ETC?
