I think the Auto-immune part of the Thyroid disease i.e IgG crosses the placental membrane. and causes the damage rather the Hormone itself. Unless the mother is hypothyroid herself and the fetus has also production issue, then there is a severe case of congenital hypothyroidism. Otherwise if the mother is normal the fetus responds to it correctly. It after birth that effects of hypothyroidism come into play. "......................They include not only maternal iodine deficiency and various congenital abnormalities of the fetal hypothalamo-pituitary-thyroid axis but also maternal antithyroid antibodies that cross the placenta and damage the fetal thyroid. T4 crosses the placenta, and unless the mother is hypothyroid, growth and development are normal until birth. If treatment is started at birth, the prognosis for normal growth and development is good, and mental retardation can generally be avoided; for this reason, screening tests for congenital hypothyroidism are routine in all states of the USA and most other developed countries.
