At autopsy, a patient who had died with acute anuria and uremia is found to have ischemic necrosis of the cortex of both kidneys with relative sparing of the medulla.These pathological findings are most likely to related to which of the following underlying conditions?

a) DIC

b) Multiple myeloma

c) Polycystic Kidney Disease

d) Pyelonephritis

e) Sickle anemia.

Still have much to learn. rolling eyes My resources don't explain this key feature of DIC. Where would I find such info?

I simply can't see the relationship between DIC and anuria. Anuria is usually caused by stones. Stones can be a complication of pyelonephritis. After looking up this topic, I'm left even more confused. Isn't this a case of renal paillary necrosis? and is not pyelonephritis a cause? What am I missing? raised eyebrow

DIC------> Heavy Bleeding ----------->dramatic drop in blood pressure---------> decrease renal perfusion-----------> Anuria

If anyone knows where i can find more information about this please let me know. After doing some more reading, I finally get it. Pyelonephritis as sequelae of renal papillary necrosis can't be correct because the medulla is not spared. I know it's probably not a suprise to many of you, but I'm relieved now because a little light bulb just went on in my head. grin
